The Puzzle of Vitiligo, Oxidative Stress and Narrow Band Ultraviolet-B

چکیده

Obviously, vitiligo is a multifactorial disease, and at this stage, we all accept that the autoimmune theory, the adhesion defect theory and the biochemical theory based on oxidative stress are the mainstays of our current knowledge about the aetiology of the disease. The presence of an imbalance in the anti-oxidant system in vitiligo melanocytes was assessed early, providing support for a free radical-mediated damage as an initial pathogenic event in melanocyte degeneration in vitiligo [1,2]. Since then, it was demonstrated that vitiligo patients had an increased epidermal level of H2O2 but also an increased superoxide dismutase (SOD) activity, especially in active lesions [3,4]. Not surprisingly, systemic antioxidants provide a helpful adjuvant in the treatment of vitiligo, as well as topical antioxidants, among them combinations of SOD and catalase (CAT) whose activity has been reported as similar to that of 0.05% betamethasone [5].

Evaluation of Efficacy and Safety of Combination of Narrow Band Ultraviolet B and Topical Calcipotriol Versus Narrow Band UVB alone in the Treatment of Vitiligo

Background: Narrow band Ultraviolet B (NBUVB) has been used successfully for the treatment of vitiligo. Recently, topical calcipotriol has also been used as immunotherapy or as a part of combination therapies. The aim of this study was to compare the clinical efficacy and safety of NBUVB and NBUVB plus topical calcipotriol in the treatment of generalized vitiligo. Methods: NBUVB phototherapy was...

Evaluation of the Efficacy of Topical Ethyl Vanillate in Enhancing the Effect of Narrow Band Ultraviolet B against Vitiligo: A Double Blind Randomized, Placebo-Controlled Clinical Trial

Background: Vitiligo is an acquired disease of skin that presents with depigmented patches due to lack of melanocytes in the epidermis. Accumulation of toxic free radicals like hydrogen peroxide in the epidermis may be responsible for melanocytes death. Since ethyl vanillate (vanillic acid ethyl ester) is a strong hydrogen peroxide scavenger, it may be effective against vitiligo. This study was...

سطح C-reactive protein در سرم بیماران مبتلا به پسوریازیس قبل و بعد از درمان با narrow-band ultraviolet B

Background and Aim: C-reactive protein (CRP) is an inflammatory biomarker and its level increases in serum of psoriatic patients. Its level is also associated with Psoriasis Area and Severity Index (PASI) score. Methods: CRP serum levels in patients with psoriasis were measured before and after treatment with narrow-band ultraviolet B (NB-UVB) and the data were analyzed in relation the PASI scor...

Antioxidants and narrow band-UVB in the treatment of vitiligo: a double-blind placebo controlled trial.

BACKGROUND Vitiligo is an acquired depigmenting disease with uncertain aetiopathogenesis, possibly associated with oxidative stress. Narrowband ultraviolet B phototherapy (NB-UVB) is the most widely used and effective treatment. AIM To evaluate the clinical effectiveness of NB-UVB and the repairing of oxidative stress-induced damage, using oral supplementation with an antioxidant pool (AP). ...

Serum level of vitamin D3 in vitiligo patients before and after treatment with narrow band UVB. Is there an association with treatment?

Background: Vitiligo is a pigmentation disorder of the skin characterized by the loss of melanocytes through different mechanisms. Narrow band UVB (NBUVB) is a safe and effective treatment for vitiligo that acts by inducing the synthesis of cytokines involved in melanogenesis. NBUVB appears to be involved in the treatment of vitiligo by increasing the synthesis of vitamin D, which prevents the ...
